PLEASE READ THE FOLLOWING carefully before signing this waiver of liability:

1. I, for myself, my child, ward, or the minor(s) named below, sign this Waiver and Release of Liability in

consideration for the opportunity. to participate in all off-premises activities. I, for myself, my child, ward,

or the minor(s) named below, agree there are risks, both known and unknown, traveling or participating in

any activity, including, but not limited to physical injury, emotional. injury, distress, paralysis and even

death. The risk of serious injury from participating in all off premise activities, although minimal, does

exist.

2. I agree, for myself, my child, ward, or the minor(s) named below, that we shall comply with all regulations,

rules, signs, and verbal instructions as condition of travel and participation in these activities and

acknowledge that failure to do so may result in immediate removal. Adults will be available during all offsite

activities.

3. I, for myself, my child, ward or the minor(s) named below, and on behalf of my or their heirs, assigns,

personal representatives and next of kin, knowingly and freely assume all risks of injury and agree to hold

harmless the East Delta Baptist Church, its owners, managers, agents and employees with respect to any

claims, demands, causes or rights of action, even if arising from the actions, acts of omission or negligence

of said entities or other guests.

4. I am at least 18 years old and represent that I am the parent or legal guardian of the minor(s) named

below, or I have obtained permission from the parent/legal guardian of the minor(s) named below to

execute this agreement on their behalf. I hereby confirm that the child(ren) are physically, mentally, and

emotionally fit to participate in such activities. I acknowledge that my, my child’s, wards and/or minor(s)

named below participation in activities in the Bounce House and all activities is strictly voluntary.
